RESUMO
The Quebec Heart Institute was established in 1957 at the Laval Hospital in Sainte-Foy, Quebec. Since then, clinical and research activities have made this Institute one of the largest tertiary care cardiology centres in Canada. With its vast catchment area of more than 3,000,000 people, the Institute has developed a strong collaboration with referral physicians centred on clinical, teaching and research interests. The Institute pioneered several aspects of cardiac surgery, invasive cardiology, echocardiography, basic research and, more recently, a network of researchers and clinicians working in the field of 'metabolic cardiology'. The first 50 years of the Quebec Heart Institute are depicted in this overview, which will also introduce this special supplement to The Canadian Journal of Cardiology.

The authors present data on the development and introduction of anesthetic techniques during cardiac surgery at the Institute of Thoracic Surgery, USSR Academy of Medical Sciences, in 1956-1960 and after its reorganization to the Institute of Cardiovascular Surgery, USSR Academy of Medical Sciences, in 1961-1965. It is shown that in the years of introduction of closed operations on the heart, the methods of one- and many component inhalational anesthesia were mastered, its techniques were developed, anesthesia apparatuses and an anesthesia schedule were designed, cardiac anesthesiological studies were conducted, training of physicians from the country's regions was initiated, and the first guidelines for general anesthesia were published. In these years, the firm foundation was laid for the development of cardiac anesthesia. Later on the Institute developed and introduces all basic types of inhalational anesthesia during operations on the open heart under both extracorporeal circulation and hypothermia. The gained experience allowed the laboratory staff to defend several dissertations, to issue two monographs, and to analyze errors and risks of general anesthesia in patients with cardiovascular diseases at surgery.

This is the second in a series of five papers that Dr. Wooley wrote exclusively for The American Heart Hospital Journal based on his book, The Irritable Heart of Soldiers and the Origins of Anglo-American Cardiology, published by Ashgate Publishing in 2002.1 Dr. Wooley's book also casts light on the origins of the specialty heart hospital.

The most significant facts in the development of Chilean cardiology since 1930 are reviewed. The preeminence of great clinicians in the thirties was followed by the creation of Departments of Cardiology in University Hospitals. The emergence of powerful diagnostic tools and cardiac surgery allowed an accelerated development of this specialty in our country. Some of the problems faced by physicians and public in general as a consequence of the increasing use of sophisticated technology are discussed.

El departamento de medicina nuclear del Instituto Nacional de Cardiología inició sus labores clínicos el 2 de enero de 1977 en el momento oportuno, cuando surgían los primeros procedimientos de la cardiología nuclear, pero coincidente con el inicio de la crisis financiera. El transito de la medicina nuclear general hacia la cardiología nuclear ha sido lento en este Instituto. Durante los primeros 8 años los estudios cardiológicos representaron únicamente el 10.8 por cento de todos los estudios in vivo, pero en 1968 ya constituyeron el 37.8 por cento . Empero la demanda creciente de estos estudios tiende a desplazar a los de otros óganos, debido a la saturación total de nuestra única cámara de centelleo. Podemos concluir que la evolución de la cardiología nuclear en este Instituto ha sido lenta, pero representa un progreso verdadero en base a la infraestrutura tecnológica creada con recursos internos.
